REQUEST FOR INFORMATION (RFI)

Crisis Residential Services

Section I.      General Information

 

Shiawassee County Community Mental Health (CMH) is a publicly funded mental health authority that provides a range of community-based behavioral healthcare services in Shiawassee County.   Services are provided through a combination of directly operated and contracted programs.    There is a main service site location through which directly operated outpatient, case management, psychiatrist, developmental disabilities day activity, and children’s home-based services are provided.  In addition to its main service site, assertive community treatment and a consumer-run drop in center service are provided out of other locations.  Contracted services include inpatient, specialized residential, and respite services to adults and children with developmental disabilities and children with serious emotional disturbances.  

 

Shiawassee CMH has been part of a multi-county affiliation, the Access Alliance of Michigan Alliance (AAM).   This alliance includes the Bay- Arenac Behavioral Health, Huron Behavioral Health, Tuscola Behavioral Health, Montcalm CMH.

 

Section II.      Background Information

 

The purpose of this RFI is to solicit information from interested Providers who may wish to provide crisis residential services in Shiawassee County.

 

Crisis residential services is to be provided in a licensed adult foster care home that has received special certification from the State of Michigan as a crisis residential unit.  Shiawassee County CMH does not have a service location, the interested provider(s) would need to pursue finding and obtaining licensure and certification for this home.

 

Section III.     Statement of Work

 

Shiawassee CMH is soliciting persons/organizations interested in providing these crisis residential services under contract with CMH.    These crisis residential services are outlined in the Medicaid Manual, see attached information.

 

 

Section IV.   Specifications for Information Requested

 

Individuals or organizations interested in providing a response to this RFI should include the following information, organized and labeled consistent with the format outlined below:

 

A.    Description of Vendor’s Training and Experiences providing crisis residential services.

Describe the vendor’s training and experiences providing crisis residential services as outlined above.     Also describe any experiences in developing crisis residential home.

 

B.                Description of Vendor’s Training and Experiences with CMHs and publicly funded mental health services in Michigan.

Describe the vendor’s training and experiences working in the publicly funded mental health service system in Michigan.   Include a description of any experiences with nonprofit boards and agencies providing crisis residential services to persons with disabilities.

 

C.               Costs

Provide information regarding the provider’s rate/charges for providing crisis residential services.

 

D.               References

Provide the name, address, and contact person for three references regarding the individual’s or organization’s ability to provide crisis residential services as outlined above.

 

PERSONS INTERESTED IN SUBMITTING A RESPONSE TO THIS REQUEST SHOULD FORWARD THE INFORMATION OUTLINED ABOVE TO SHIAWASSEE CMH SERVICES, ATTN. CONTRACT MANAGEMENT, 1555 INDUSTRIAL DRIVE, PO BOX 428, OWOSSO,  MI, 48867, NO LATER THAN MAY 13, 2008.   RESPONSES MUST BE RECEIVED WITHIN THE CMH OFFICES NO LATER THAN 3:00 P.M. ON THAT DATE.   RESPONSES RECEIVED AFTER THAT TIME WILL NOT BE ACCEPTED.

 

Section V.    General Information Regarding the RFI Process

 

A.                RFI Response(s) Review Process

A committee of CMH representatives will review all responses to this RFI.   Providers may be asked to provide additional information to members of this committee to assist in the evaluation of Providers’ responses.

 

The review committee will evaluate each response based on the provider’s training and experience, references, and the cost information provided.

 

B.                Rejection of Responses/Contract Awards

CMH reserves the right to reject any and all responses received as a result of this RFI.    This RFI has been developed for information and planning purposes, and may not lead to contracting opportunities.

 

Should this RFI process lead to the development of a service contract(s),CMH reserves the right to develop contract terms and conditions which may be different from the information provided in the RFI responses.

 

C.               Incurring Costs

CMH is not liable for any cost incurred by the Providers prior to issuance of a contract.

 

D.               Disclosure of RFI Response Contents

Responses are subject to disclosure under the Michigan Freedom of Information Act (FOIA) (P.A. 1976, No. 442), once the RFI cycle is complete.   After a contract has been awarded, CMH may release copies of the RFI responses consistent with FOIA requirements.
