ANNOUNCEMENT:  2008 FUND RAISER   

 

This announcement is on behalf of the consumers of Shiawassee County Community Mental Health Authority.  Since 10/01/03 Medicaid has made major policy changes and it has altered the way that the consumers of SCCMHA receive services.  For a period of time all dental services were eliminated but now are provided on an extremely limited basis.  Medicaid has also raised the prescription drug co-pays.  Unfortunately, most of the consumers that we provide services to are mentally/physically disabled and receive Medicaid.  To assist these consumers we would like to take this opportunity to invite you to help us with our 5th Annual SCCMHA Golf Scramble.  Our goal is to prevent as many health problems as possible for our consumers.  We have set up a Consumer Health Fund to assist consumers with their medical costs.

All proceeds from the golf scramble will go directly to the SCCMHA Consumer Health Fund.

 

We respectfully ask Your Organization to assist us in one of a few ways:

§  Silent Auction Gifts

§  Golfing items for door prizes and contest holes

§  Food and Beverages

§  Provide a four-some to golf

§  Sponsor a hole ($225.00 minimum).  Make checks payable to:  SCCMHA Consumer Health Fund.  Please note hole sponsor on check.

 

As a reminder, any items that you donate to the golf scramble can be used for tax purposes for yourself and your business (SCCMHA is a non profit organization).  Our tax-exempt number is 38-2790631.
